Glass City Behavioral Healthcare was established in 2018 with the mission of providing a bridge of specialized care to children who require additional academic support due to a behavioral diagnosis. Since then, we continue to provide exceptional quality of care to our children and their families that makes a positive impact on their quality of life. Educational and social growth directives and activities that promote and encourage success in all aspects of development remain our goal. Now Hiring: Case Manager-Part Time
Position Includes:
The Case Manager will manage cases for their assigned clients to ensure that each client receives services needed for overall well-being and in accordance with his/her individualized service plan (ISP).
Responsibilities:
- Counsel and advise individuals or groups with Behavioral issues.
- Develop and implement individual treatment plans.
- Establish treatment goals and milestones with clients.
- Prepare progress reports and letters for relevant parties.
- Educate families and groups about treatments and prevention programs.
- Facilitate referrals to other healthcare or counseling professionals.
- Keep accurate and Medicaid documentation.
- Complete group therapy
- Other duties assigned.
Qualifications:
- Previous experience in case management, social work, or other related fields
- Experience working with an Electronic Health Record System of some sort
- Experience completing Medicaid Billable progress notes
- Experience in conflict resolution
- Compassionate and caring demeanor
- Ability to build rapport with clients
- Excellent written and verbal communication skills
- Must have transportation, valid license with less than 4 points on it and insurance
- Minimum of High School Diploma or equivalent
- Bachelors’ degree preferred, but not required.
This is for our afterschool program. Hours would be from 2:45pm until 7:30pm Monday through Thursday.
